Paper 35: Preoperative Predictors of Arthroscopic Partial Meniscectomy: The APM Index Score

چکیده
Objectives: Meniscal tears are one of the most common knee conditions treated by orthopaedic surgeons. Patients who do not respond to conservative management often offered arthroscopic partial meniscectomy (APM). Clinical care these patients could be enhanced a scoring system that uses preoperative factors predict likelihood pain relief following APM. The purpose this study is create an index score based on easily available risk favorable outcomes Methods: We performed retrospective review prospectively collected data from consecutive cohort single surgeon in outpatient setting. were included if they had meniscal tear, failed nonoperative treatment measures, and completed patient reported outcome questionnaires. defined clinical improvement as achieving ≥ 90 at one-year postoperatively or 10 points KOOS subscale (the minimal clinically important difference (MCID)). used multivariable logistic regression models identify associated with created assigning each variable proportional its coefficient. Results: 468 underwent APM between August 2012 October 2020 study. mean age was 49 [SD=10.4, range 19-81], 58% female, 88% white, 79% overweight obese. 84% subjects met our criteria for improvement. In model, longer symptom duration, higher Kellegren-Lawrence (KL) grade, pre-treatment lower postoperatively. adjusted odds ratios shown Table 1. Based coefficients those KL grade 0 assigned 2 all other (pre-treatment ≤ 60, duration 6 months, 1 2) point (Table 1). algorithm well total scores predicting improvement: 40% 0, 68% 1, 80% 2, 89% 3, 96% 4 (Figure also examined did include since it might readily abbreviated < 3 months points; 3-6 ranged 66% among 90% Conclusions: results serve screening tool may help would benefit intervention. Further research should validate independent sample. note does compare benefits undergoing versus treatments, but can physician engage shared decision-making process regarding possible [Table: see text]

Arthroscopic , Open Partial , and Total Meniscectomy
The results of three different types of meniscectomy have been compared in 219 knees, 71 treated by arthroscopic partial meniscectomy, 45 treated by open partial meniscectomy, and 103 treated by open total meniscectomy, with a mean follow-up of 4.3 years. Knees which had undergone previous operations or had other simultaneous operative procedures or ligamentous damage were excluded. Knees with ...
متن کاملDoes arthroscopic partial meniscectomy always cause arthritis?
The goal of arthroscopic partial meniscectomy is to preserve a stable rim of meniscal cartilage, removing only unstable, diseased tissue. As one of the most common orthopedic procedures performed in the United States, arthroscopic partial meniscectomy developed as an alternative to open total meniscectomy that leads to degenerative changes in the knee over time. The short-term results of this m...

To the Editor: Previous studies have suggested that partial meniscectomy has little benefit in patients with advanced osteoarthritis1,2 but some benefit in those with mild-to-moderate osteoarthritis.3 Sihvonen and colleagues (Dec. 26 issue)4 attempted to assess the benefit of partial meniscectomy in patients without osteoarthritis. However, although the authors excluded patients with radiograph...
متن کاملOsteonecrosis of the Knee after Arthroscopic Partial Meniscectomy
Osteonecrosis of the femoral condyle is known as an uncommon complication after arthroscopic meniscectomy. The lesion of osteonecrosis can be irreversible, thus early detection of the disease is crucial for treatment. A 50-year-old male patient without known risk factors of osteonecrosis developed increasing knee pain after arthroscopic partial meniscectomy. Magnetic resonance imaging showed ra...

In an open prospective study the results of arthroscopic partial meniscectomy were reviewed in 188 knees. Group I comprised of 139 (65%) knees with pure meniscus lesions without any ligament laxity, the remaining 49 (35%) knees in group II had anterior cruciate ligament (ACL) deficiency in addition to meniscus lesions. In group I, 93% had excellent to good and 7% had fair results by criteria of...
